Dr Perko consults at The Mater (North Sydney), The Stadium (Moore Park) and Wollongong.

All new patients are required to complete a Patient Registration Form prior to their consultation with Dr Perko.


When you come for your appointment please remember to bring the following:

  • Referral letter from your GP, family physician or other doctor
  • Scans and reports – i.e. X-rays, MRI’s, CT’s, Injections etc.
  • Medicare card and Private Health Insurance details
  • DVA or Pension card if applicable
  • Any other relevant information to your injury or condition
  • A completed Patient Registration Form
All Work cover patients are required to also bring an approval from their insurer for the initial consultation. This can be faxed or emailed to us prior to the appointment.

If you need to cancel your appointment please call the office during business hours and notify the staff. Please allow at least 1 day’s notice so that we can offer your appointment to another patient.


Payment is due at the time of the consultation. When you make an appointment the secretary will let you know the cost of the consultation.


New patient consultations generally run for 20 minutes. If you are a returning patient these consultations generally run for 10 minutes. We do our best to run to time. Unfortunately emergencies do occur which may cause delays. We appreciate your patience if we are running behind.


All new patients are required to complete patient registration forms. Please complete the relevant details prior to your appointment with Dr Perko to save time on the day of your appointment.

If you would prefer you can print out the forms and bring the completed forms on the day on your consultation.
